Form 1095-C Notice
In accordance with the Paperwork Burden Reduction Act (Public Law 118-167) and IRS guidance, Trintech, Inc. is providing this notice regarding Form 1095-C, Employer-Provided Health Insurance Offer and Coverage.
Employees and former employees who were eligible for Trintech’s medical coverage may request a copy of their Form 1095-C by contacting:

Requested forms will be provided within 30 days of receipt or by January 31, whichever is later.
Trintech continues to file all required Forms 1095-C with the IRS and will comply with any state-specific distribution requirements.
